1) Photocopy of NSO Birth Certificate
2) Photocopy of Baptismal Certificate or
certification of affiliation in a religious group (for Non-Catholics)
3) Four (4) copies of 1x1 colored I.D.

Elementary Levels: Grades
1 to 6

Other Requirements for Foreign Students / Non-Filipino
Citizens
1) Photocopy of Alien Certificate of
Registration (A.C.R.)
2) Study permit issued by the Bureau of
Immigration (B.I.D.)
3) Photocopy of student’s and parent’s
passport number, date of issue, place of issue and type of visa (bring original
passports for verification)
4) Students with dual citizenship must
submit the Identification Certificate as a Filipino Citizen issued by
the B.I.D.
