APPLICATION & ADMISSION REQUIREMENTS

 

Primary Levels: Junior Casa 1; Junior Casa 2; Senior Casa; Preparatory

1)    Photocopy of NSO Birth Certificate

2)    Photocopy of Baptismal Certificate or certification of affiliation in a religious group (for Non-Catholics)

3)    Four (4) copies of 1x1 colored I.D. photo

4)    1 recent family photo

5)    For all children with previous schooling – original or certified true copy of report card

6)    Medical certificate of good health

7)    1 long brown envelope

8)    Interview and Testing Fee

 

Elementary Levels: Grades 1 to 6

1)    Photocopy of NSO Birth Certificate

2)    Photocopy of Baptismal Certificate or certification of affiliation in a religious group (for Non-Catholics)

3)    Four (4) copies of 1x1 colored I.D. photo

4)    1 recent family photo

5)    Original or certified true copy of report card (no grade lower than 81 in any subject and in Conduct)

6)    Certificate of Attendance with School Permit Number

7)    Certification of good moral character by the Principal and Homeroom Adviser

8)    Medical certificate of good health

9)    1 long brown envelope

10) Interview and Testing Fee

 

Secondary Levels: Years I - IV

1)    Photocopy of NSO Birth Certificate

2)    Photocopy of Baptismal Certificate or certification of affiliation in a religious group (for Non-Catholics)

3)    Four (4) copies of 1x1 colored I.D. photo

4)    1 recent family photo

5)    Original or certified true copy of report card (no grade lower than 85 in any subject and in Conduct)

6)    Certificate of Attendance

7)    Certified true copy of Government Recognition Permit (Year II – IV transferees)

8)    Certification of good moral character by the Principal and Homeroom Adviser

9)    Medical certificate of good health

10) 1 long brown envelope

Other Requirements for Foreign Students / Non-Filipino Citizens

1)    Photocopy of Alien Certificate of Registration (A.C.R.)

2)    Study permit issued by the Bureau of Immigration (B.I.D.)

3)    Photocopy of student’s and parent’s passport number, date of issue, place of issue and type of visa (bring original passports for verification)

4)    Students with dual citizenship must submit the Identification Certificate as a Filipino Citizen issued by the B.I.D.
